I'm not sure if that will be enough.
If you have copies of receipts and such from buying SQ then it might help them track the account. Otherwise I'm not sure what other info you can give.

Can they locate your account with just your master name, servants, master lv and device last played on.

Have you ever purchased SQ? If you can show proof of purchase, maybe multiple copies and also the details regarding your account as you listed, it'll be possible. No guarantees. I've seen it work on other various mobile game platforms that I play on other forums.
If you have your transfer number written down, or friend code, that would help too.
That's a pretty thin amount of information. *Could* they locate your account with just that information? Probably, but my guess is that it would be a pain in the ass, so you're going on the bottom of their priority list.
Plus, the only info you gave them is publicly available stuff + 50/50 chance on Android/iPhone, which means they're going to wait X amount of time to make sure this is a truly inactive account, not one you're trying to steal.
Do you have any RL friends who play, who might have your friend code (or have you ever posted it online when looking to add friends)? That would at least make it easier for them to ascertain that they were looking at the right account.
You don't even have the listed birthday?
i also lost my acount once, but i didnt sent my email to fate go.us,instead to aniplex asia. they answer my email around 5 hour after i sent my email.
I got a lot of good feedback about account recovery on a question I posted here recently.

Not only did the respondents give me pointers on how to avoid losing my account, they also linked me to some comprehensive resources for recovering it should the worst happen. Most of that has been covered by people replying to you so far, but I figure it couldn't hurt to have more than one solid answer.
Best of luck.
